Benefits

Our Group Benefit package is available after successfully completing a probationary period of 5 months or 500 hours, which ever comes first. The premiums are paid 100% by the employer. If you wish to join, please download and complete the enrollment form as soon as possible and forward to the Business Manager for processing.

LONG TERM DISABILITY (Option L1)
Benefit equals 60% of Monthly Earnings up to $6,000 per month.
Benefits are payable from the 121st day of an Accident or Sickness up to age 65.

 

HEALTH (Option E3)
Nil Deductible
80% Coverage of eligible expenses

TRIP CANCELLATION INSURANCE (Option X1)
Up to $5,000 in cancellation costs

 

DENTAL (Option D11)
Nil Deductible
80% coverage of Basic services
80% coverage of Restorative procedures
50% coverage of Major & Orthodontic services

For late applications, you must also fill out this form as proof of insurability.

These forms are used when your health or dental office cannot submit electronically to process your claim.

Upon qualifying for benefit coverage the above forms may be completed and submitted to Rachel (Business Manager).  Should you have any questions do not hesitate to contact Rachel.
